Translation help for paper

[RM1704]

Hello

 

I got a new sword, and it came with this sort of origami. I haven't seen anything like this before. Do you know, what it might be, and what it says.

I think it's dated showa 48 (1973) and looks a bit like the type of "authentication" you might get from the shop where it was originally purchased. Seems to have much of the same information as a torokusho.

The mei bun - jidai column gives it to den Jumyo from around Kanbun. 

[Bugyotsuji]

Dated Showa 48, but the photo is just too fuzzy for me to make out the shop name. The proprietor signing this authentication certificate was Ōzawa Toshio.(sp?)

The shop name is 都幾川美術 - Tokigawa art.
