but what does "koshiyama Motomu kore" mean?

If Koshiyama is the correct reading of his family name 越山, it means '(Mr) Koshiyama placed the order for this' or (Mr, etc.) 'Koshiyama requested/ordered this'.

 

Motomu is from the verb motomeru, 'look for' 'ask for' or 'want', and kore is 'this'.
