Surrender Tag can anyone help with a translation?

On the Yoshikuni Shinto Katana I posted in the Nihonto forum it had this surrender tag attached. It's very faded and hard to read. Can anyone help with a translation? I'm curious as to what it says.

if u blacklight it the ink should show up more clear.......

I tried but it didn't really come out any better. Underneath the army kanji it looks like a bit like a Ju and then after that number 30. I just cant get the other 2 kanji which presumably correspond to a regiment or something.
