彫 御神号 (Hori Goshingo) - Carving: gods

表 八幡大菩薩 - Omote: Hachiman dai-bosatsu

裏 春日大明神 - Ura: Kasuga dai-myojin

 

裏銘 天文三年二月吉日 - Ura-mei: Tenbun 3rd year, a lucky day in 2nd month

 

鐵山識 - Tetsuzan wrote.

(However, the first kanji is my guess and I could be wrong.)
