I think you probably already figured out 95% of it, but I put it here for people who want the whole thing.

From left to right

伯耆大原安綱

Hōki Ōhara Yasutsuna

鑑定

Appraisal

(スリ)上ゲ無銘長サ貮尺二寸六分

Suriage Mumei Nagasa 2 shaku 2 sun 6 bun

神代総焼刃作

Kamiyo Sōyakiba saku

I'm slightly guessing at suriage, but it looks to me like there is a リ just above the 上ゲ, and it seems the most logical candidate, without having seen the blade.

The last line Kamiyo Sōyakiba saku is something that also requires a bit of guesswork for me. Kamiyo (literally: age of gods) refers to the pre-recorded antiquity of Japan's history, from which Yasutsuna emerges.

Sōyakiba = completely tempered blade(?).

So in all, it looks like the sayagaki is selling the blade as a Yasutsuna blade, supposedly from the 800s.
