The two far left lines look like the location and the name of the owner

所有者福囗囗(眺?)

Shoyūsha Fuku ? ?

Owner: Fuku something-something

Nishihara Gen (personal name)

 

I don't know the significance of the 眺 kanji. At looks like it ought to be part of the person's name, but somehow it doesn't fit as a name. 

The location name starts with 福. The second kanji looks like 弓 and the third I would guess is 町, but there is no such location as 福弓町. It might be some shorthand version of 福島県 (Fukushima Prefecture) but that is starting to stretch credibility both from a handwriting perspective and a context perspective.

Actually, looks more like a price tag than a surrender tag.

If I'm not mistaken the first line is a price 

 

囗囗金六百五拾円也 650 yen

無名

 

~650 yen

mumei

 

If you could change the angle to get a better shot of those first two kanji, it might help.
